I think it can be excellent with a designer and show concept that goes beyond literalism, yes! Your 1980s highschool version sounds excellent.
In the production for which the icon's head was made, the research imagery for the ass-head was from Goya's Los Caprichos, particularly this image here:
...and the director wanted it to be as realistic as possible, "like a taxidermy donkey head." Hence, yeah. Ears on a top hat would have been much quicker in the building, that's for sure!
